What We're Looking For
- 2+ years of Care Management experience required
- ECM/CalAIM experience strongly preferred
- Spanish-speaking required
- Strong understanding of healthcare and community resources
- Excellent communication, organization, and documentation skills
- Comfortable working independently and as part of a team
- Experience with electronic health records and care management software
- Willingness to travel throughout San Francisco and the Peninsula

J&M Homecare Services is seeking an experienced Enhanced Care Management Specialist to support individuals with complex medical and social needs throughout San Francisco and the Peninsula.
In this role, you'll build trusting relationships with clients and help them navigate the systems and resources they need to live healthier, more stable lives.
What You'll Do
- Assess medical, social, and care needs
- Create and manage individualized care plans
- Coordinate with clients, families, healthcare providers, and community partners
- Connect members to healthcare, housing, transportation, and social services
- Follow up on services and track progress
- Advocate for clients and help remove barriers to care
- Provide education and support
- Maintain accurate, timely documentation
