Are you an experienced Care Manager who is passionate about helping people with complex needs navigate the healthcare system?
J&M Homecare Services is looking for a compassionate, organized, and motivated Enhanced Care Management (ECM) to join our team serving San Francisco and the Peninsula.

What You'll Do
- Assess clients' medical, social, and care needs
- Develop and manage individualized care plans
- Coordinate services between clients, families, providers, and community organizations
- Connect clients to housing, transportation, healthcare, and social services
- Monitor progress and provide ongoing follow-up
- Educate and support clients in managing their healthcare needs
- Maintain accurate and timely documentation
- Advocate for clients and help remove barriers to care
What We're Looking For
- 2+ years of Care Management experience
- ECM/CalAIM experience strongly preferred
- CHW training or certification preferred
- Strong knowledge of healthcare and community resources
- Excellent communication and organizational skills
- Compassionate, patient, and solutions-oriented
- Able to work independently while collaborating with a team
- Comfortable using electronic health records and care management software
- Willingness to travel throughout San Francisco and the Peninsula
