At Amplar Health, part of the Medibank Group, we're redefining healthcare by bringing As part of Amplar Health's growing Support at Home service, you'll be working within a contemporary, clinically-led model designed to help older Australians remain independent, mobile and connected in their own homes for longer. Our approach is centred on prevention, early intervention and reablement, helping clients maintain function and quality of life rather than simply responding when health issues arise.
You'll work alongside Clinical Care Partners, other allied health and nursing professionals, as well as Amplar Health's broader primary care network to deliver coordinated, person-centred care. This integrated approach provides a unique opportunity to influence health outcomes across the entire care journey and make a meaningful difference to how older Australians experience ageing at home.You'll thrive in this role if you're a compassionate and adaptable Registered Nurse who enjoys working with autonomy, problem-solving, and providing person-centred care.
What we're looking for:
- Current unrestricted AHPRA registration as a Registered Nurse
- Minimum 2 years' nursing experience in Aged Care and Community settings
- Strong clinical assessment and care planning skills within Aged Care
- Live in or near North Gold Coast areas such as Arundle
- Excellent communication and relationship-building abilities
- A passion for delivering outstanding patient experiences
