Role Description

● Provides independent, end-to-end hemodialysis care for assigned patients, ensuring adherence to DOH, PSN, and PhilHealth standards.

● Serves as a clinical resource for less-experienced nurses and trainees, demonstrating proficiency in technical, safety, and documentation practices.

● Supports quality assurance (QA), infection-control, and audit processes that sustain the clinic’s regulatory compliance and service quality.

Key Responsibilities

● Perform comprehensive HD sessions independently—assessment, initiation, monitoring, termination, and post-treatment care for stable and moderately complex patients.

● Manage all vascular access types (AVF, AVG, CVC) with demonstrated competence in aseptic cannulation, patency checks, and complication management.

● Recognize, respond to, and document complications such as hypotension, disequilibrium, infiltration, and access issues using established protocols.

● Ensure documentation accuracy for each session, including HD flow sheets, treatment summaries, and PhilHealth claim data for audit readiness.

● Collaborate with nephrologists and biomedical staff to coordinate care, troubleshoot alarms, and maintain machine functionality and water-quality compliance.

● Supervise and coach Trainee Nurses, providing structured feedback and reinforcing adherence to clinical standards.

● Supports clinic-level inventory control by performing assigned checks on emergency carts, consumables, and dialysis supplies, and promptly escalating discrepancies to the Nurse Manager.

● Participate in internal audits and QA activities, contributing data to performance indicators (infection rates, incident trends, treatment completion).

● Maintain infection-control integrity, including isolation room procedures, reprocessing validation (if applicable), and disinfection protocols per DOH guidance.

● Contribute to patient education and engagement, reinforcing treatment adherence, vascular access care, and symptom reporting.

● Monitor stock and consumable use, reporting discrepancies and supporting efficient inventory management.

● Engage in continuing education, RENAP activities, and skill refreshers to maintain current practice standards.

● Demonstrate professional leadership through teamwork, accountability, and proactive contribution to clinic operations.
