Job Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) Care Coordinator to join our assisted living community. In this role, you will be responsible for providing compassionate nursing care, coordinating resident health plans, and ensuring the delivery of high-quality services tailored to each individual's needs. Responsibilities
- Provide direct nursing care to residents in accordance with established care plans and facility policies.
- Assist with daily living activities, medication administration, and health monitoring for residents.
- Collaborate with healthcare providers, families, and team members to develop and update personalized care plans.
- Serve as a primary point of contact for residents' health concerns, ensuring timely intervention and appropriate responses.
- Document resident health status, interventions, and outcomes accurately and thoroughly.
- Educate residents and their families about health conditions, medications, and wellness strategies.
- Coordinate medical appointments and follow-up care to ensure comprehensive health management.
- Ensure compliance with all regulatory requirements and safety standards within the assisted living environment.
Skills
- Valid Licensed Practical Nurse (LPN) license with current certification.
- Experience working as an LPN in an assisted living or similar healthcare setting preferred.
- Strong understanding of elderly care needs and assisted living operations.
- Excellent communication skills to effectively interact with residents, families, and healthcare team members.
- Ability to assess resident conditions accurately and respond appropriately.
- Compassionate approach with a focus on resident-centered care.
- Organizational skills to manage multiple residents' needs efficiently.
- Knowledge of healthcare regulations and compliance standards relevant to assisted living facilities.

Pay: $25.00 - $28.00 per hour
Expected hours: No less than 40.0 per week
