InCare Ohio Home Health is looking for a Full time RN or LPN for our Medication Management in Malvern! 12 hour shifts doing home health visits in one facility! Hourly pay with no travel.

Overview

Join our dedicated home health team as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) specializing in medication management! Duties

- Administer medications accurately and safely according to physician orders, including injections, IV infusions, and oral medications.

- Conduct thorough patient assessments, including vital signs, physical examinations, and medication reconciliation.

- Develop and implement individualized care plans tailored to each patient’s needs, with a focus on medication management and safety.

- Educate patients and their families on medication use, potential side effects, and adherence strategies to promote optimal health outcomes.

- Monitor patient responses to medications, document progress meticulously within Electronic Health Record (EHR) systems such as Epic or Cerner.

- Coordinate with interdisciplinary teams for complex cases involving hospice & palliative medicine, trauma medicine, or behavioral health needs.

- Ensure compliance with HIPAA regulations and maintain accurate medical documentation for all patient interactions.

Skills

- Valid LPN or RN license with current state licensure.

- Proficiency in EHR/EHR systems like Epic or Cerner for documentation and medication tracking.

- Experience with medication administration techniques including injections, IV infusions, catheter care, and aseptic technique.

- Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to home health care.

- Familiarity with complex medical conditions such as Alzheimer’s/dementia care, long-term care, memory care, or acute pain management.

- Ability to perform comprehensive patient assessments and triage effectively in a home setting.

- Excellent communication skills for educating patients and collaborating with healthcare teams across specialties like cardiology, nephrology (dialysis), infectious disease care, and geriatrics.
