Program Nurse

OPWDD Credential Required
This position requires an active OPWDD (Office for People With Developmental Disabilities) credential in good standing, in addition to current New York State licensure and registration.
Qualifications
● Must be currently licensed and registered by the New York State Department of Education (RN or LPN as applicable).
● Active OPWDD credential required.
Scope of the Role
Working under the general supervision of the Program Director and the clinical supervision of the Supervising Nurse, the Program Nurse is an active member of the Circle of Support. This role identifies and assesses each individual's personal hygiene practices and skills, and develops and implements strategies to help individuals reach their maximum level of independence in health, hygiene, and grooming.
Job Responsibilities
● Provide oversight of program participants' health.
● Supervise and train Direct Support Professionals in all delegated nursing tasks.
● Develop Plans of Nursing Service.
● Monitor individuals for signs and symptoms of acute illness and complications or exacerbations of chronic illness.
● Monitor the effects of medication on individuals' ability to participate in activities.
● Administer medications and treatments as needed.
● Consult with health care providers from other agencies and with individuals' families to ensure continuity of care.
● Provide health, nutrition, and personal hygiene counseling to individuals.
● Ensure adequate supply of medication, medical, and nutritional supplies.
● Collaborate with the Circle of Support in developing treatment plans.
● Participate in on-call rotation.
● Perform other program- and Agency-related tasks as directed by supervisor/designee.
● Maintain work schedule, punctuality, and availability for overtime as requested.
● Work cooperatively with others, comply with all Agency policies and procedures, and maintain a professional demeanor at all times.
