Job Summary
We are seeking a dedicated and compassionate Registered Nurse to join our dynamic Home Health Team. In this vital role, you will provide high-quality patient care across diverse settings, ensuring safety, comfort, and optimal health outcomes. Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories.
- Provide education to patients and families on proper home health care procedures and strategies.
- Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
- Utilize electronic health record (EHR) systems such as Axxess to document patient information meticulously and ensure compliance with HIPAA regulations.
- Manage complex clinical procedures including IV Abx, Wound vac's, Picc site care
- Educate patients and their families on health conditions, medication regimens, discharge instructions, and wellness strategies tailored to individual needs like geriatrics, pediatrics, or individuals with developmental disabilities.
Qualifications
- Valid RN license in good standing with current state requirements.
- Proven experience working in diverse healthcare environments such as hospitals , nursing homes, assisted living facilities, or home care settings.(Home Health Preferred)
- Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/ICD-10), and CPT coding for accurate documentation and billing processes.
- Proficiency with EMR/EHR systems such as Epic, Axxess; familiarity with laboratory testing procedures and specimen collection is preferred.
- Demonstrated expertise in critical care procedures including airway management, trauma medicine, infectious disease care, and acute pain management.
- Experience working with specialized populations such as memory care residents, or individuals with disabilities or dementia.
- Ability to perform advanced clinical skills like phlebotomy, venipuncture, ostomy care, dialysis support, and respiratory support techniques such as ventilator management.

Pay: $44.28 - $53.33 per hour
