Job Title: Pharmacist

Location: Nashville, TN 37236.

Work Arrangement: On-site.

Employment Type: Contract.

Duration: 13 Weeks (Extendable).

Schedule: Days/Evening Rotating, every other weekend.

Domain: Healthcare

Pay Rate: $75/hr to $80/hr.

POSITION OVERVIEW:

The Pharmacist is responsible for providing safe, accurate, and effective medication management for patients within the hospital setting.

This role works collaboratively with physicians, nurses, pharmacy technicians, and other members of the interdisciplinary care team to optimize medication therapy and promote positive patient outcomes.

ROLES & RESPONSIBILITIES:

- Review and verify medication orders for accuracy, appropriate dosing, allergies, interactions, contraindications, and therapeutic effectiveness.

- Dispense medications in accordance with physician orders, hospital policies, and state and federal regulations.

- Provide clinical recommendations regarding medication selection, dosing, monitoring, and adjustments.

- Monitor patient medication therapies, laboratory results, renal/hepatic function, and other relevant clinical information.

- Participate in medication reconciliation and support safe transitions of care.

- Prepare, verify, and oversee sterile and non-sterile medication compounding as applicable.

- Provide oversight and guidance to pharmacy technicians and support staff.

- Respond to medication-related questions from physicians, nurses, patients, and other healthcare professionals.

- Participate in emergency response situations, including code blue and other critical patient events as required.

- Maintain accurate medication records and documentation within the electronic medical record.

- Ensure compliance with controlled-substance regulations, medication storage requirements, and hospital pharmacy policies.

- Assist with medication inventory management, shortage mitigation, and formulary compliance.

- Identify, document, and help prevent medication errors and adverse drug events.

- Provide patient and caregiver education regarding medications when appropriate.
